Efficacy and Safety Assessment of Antifungal Sequential Therapy from Micafungin to Liposomal Amphotericin B for Antibiotics-Refractory Febrile Neutropenia in Patients with Hematologic Malignancies

Invasive fungal infections are a major challenging problem in the management of febrile neutropenia (FN) patients with hematologic malignancies. Liposomal amphotericin B (L-AmB) or micafungin (MCFG) has been widely used as first-line empirical antifungal therapy for suspected infection such patients. However, there several issues receiving these agents: drug related toxicities L-AmB and breakthrough MCFG. In order to make best use 2 agents, we conducted prospective study sequential from MCFG L-AmB, evaluated efficacy safety this strategy FN A total 18 were enrolled, 11 who fulfilled protocol defined criteria evaluated. Underlying diseases consisted acute leukemia (n = 9), non-Hodgkin lymphoma 1), myelodysplastic syndrome 1). Treatment success was achieved 8 (72.7%). Drug-related adverse events occurred All those except one case below grade 2. Three required discontinuation L-AmB. Although our seems be encouraging antibiotics-refractory malignancies, further investigation large-scale studies is warranted.
